Professional Coordination Request
Send a Coordination Request
Send the trip details and our local dispatch team will follow up to confirm what we can cover. Submitting this form is a request, not a confirmed trip.
Need to coordinate now? Our local dispatch line is staffed 24/7.
Straight Answers Before You Send
What is this form for?
It is the professional intake for anyone arranging transportation on someone else's behalf — case managers, discharge planners, workers' compensation coordinators, claims professionals, facility staff, social workers and law-firm staff. Send the trip details and our local Dallas-Fort Worth dispatch team follows up to confirm what we can cover.
Does sending this request mean the ride is scheduled?
No. It is a request. Nothing is committed until Presence NEMT confirms availability, geography, mobility level and timing with you. We do not want you promising a patient or a family a ride we have not confirmed.
What if I need to coordinate right now?
Call or text local dispatch at (817) 900-6368. Our dispatch line is staffed 24/7 and is faster than a form when the timing is tight. Presence NEMT is not an emergency ambulance service — call 911 in an emergency.
How much information should I include?
Logistics only. Pickup, destination, appointment time, mobility level, whether it repeats, and anything about transfers, chair type or access that affects which vehicle we send. Please leave out diagnoses, records, Social Security numbers and insurance ID numbers.
Are patients and families supposed to use this?
Patients and families are welcome to call or text, or to use the standard transportation request instead. This page is built for the professional arranging the ride for someone else.
What Dispatch Actually Needs
The person arranging the ride is almost never the person riding, so the fastest coordination happens when the request already contains what dispatch would otherwise call you back to ask.
- Pickup address or facility, including the unit or entrance if it matters
- Destination and the appointment or requested arrival time
- One way, round trip, or a standing recurring series
- Mobility level, and the chair type and approximate weight if applicable
- Whether a family member or an escort is riding along
- Transfer or positioning considerations relevant to the vehicle
- Who to contact with questions, and who should receive trip confirmation
Trip Types Coordinated Through This Form
Referral volume clusters around discharges and repeat treatment. These are the trip types set up most often:
- Hospital discharge to home, skilled nursing, rehab, assisted living or memory care
- Facility-to-facility transfers
- Physical and occupational therapy, including recurring plans of care
- Orthopedic, post-operative and pain management appointments
- Occupational medicine and return-to-work visits for injured workers
- Imaging, diagnostics, dialysis, oncology and infusion schedules
- Long-distance and interstate medical transportation, quoted per trip
Boundaries We State Plainly
Clear boundaries make coordination easier for everyone involved.
- Presence NEMT is not an emergency ambulance service. Call 911 in an emergency.
- We transport medically stable riders who do not need monitoring or clinical intervention en route.
- Whether a rider is fit to travel is a decision for the treating clinician, not for us.
- We do not claim any insurer, TPA, employer or network contract. Ask us about payment arrangements directly.
- We do not publish a guaranteed response or lead time. Tell us the date and time you are working with and we will confirm what we can cover.
Frequently Asked Questions
- How do I request non-emergency medical transportation for a patient or client?
- Send the coordination form on this page with the pickup, destination, appointment time, mobility level and whether the trip repeats, or call or text local dispatch at (817) 900-6368. Presence NEMT confirms what it can cover before anything is scheduled.
- Can I set up a recurring series rather than booking each visit?
- Yes. Choose 'Recurring series' and describe the days, times and expected duration. Dispatch sets it up once as a standing series instead of visit-by-visit bookings.
- Can I request a CNA, MA or Nurse escort with the ride?
- You can request one. An escort is not an included part of the ride — it depends on the rider's needs and staffing availability and is quoted separately.
- Which mobility levels can Presence NEMT transport?
- Ambulatory riders, manual and power wheelchair riders, bariatric riders with appropriately rated equipment, and riders who need BRODA Chair or Traversa reclined positioning support as a stretcher alternative.
- Should I include medical information in the form?
- No. Keep it to transportation logistics. If anything further is genuinely needed to move the rider safely, dispatch will request it directly through the appropriate channel.
- Which areas does Presence NEMT serve?
- Dallas-Fort Worth and approved surrounding areas, including Tarrant, Dallas and Wise counties — Fort Worth, Dallas, Arlington, Irving, Haslet, Keller, Southlake, Grapevine, Colleyville, North Richland Hills, Hurst, Bedford, Euless, Roanoke, Watauga and neighbouring communities.
