Straight Answers

When does a hospital discharge need an ambulance instead of NEMT?

When the patient requires active medical monitoring, oxygen management, IV therapy or clinical intervention during the ride, or is not medically stable, that discharge needs an ambulance. Presence NEMT is not an emergency ambulance service; call 911 for a medical emergency.

How far ahead should discharge transportation be booked?

Call or text as soon as discharge looks likely, even before the exact time is confirmed — an approximate window is enough to get started, and same-day requests are routine.

What happens when the discharge time slips?

Keep dispatch posted and the pickup is adjusted. Because dispatch is local and staffed 24/7, a shifted discharge time is handled with a phone call rather than a new booking.

A Ride Home That Does Not Hold Up the Discharge

Presence NEMT provides hospital discharge transportation throughout Fort Worth, Dallas and the surrounding DFW communities. When a patient is cleared to leave and the family cannot manage the trip — a wheelchair, a fresh surgical site, oxygen, a long walk to the parking garage — we take that piece off the table.

Discharge is rarely predictable. Orders come through mid-afternoon, transport gets called late, and the bed is needed. Our dispatch is local and staffed around the clock, so a same-day discharge request gets a straight answer about what time we can be there instead of a maybe.

Destinations After Discharge

If the destination is unfamiliar to the family, we handle the address details, gate codes and entrance logistics rather than leaving a patient sitting in a lobby.

  • Hospital to home, including apartments, houses and independent living
  • Hospital to a skilled nursing facility for short-term rehabilitation
  • Hospital to an inpatient or outpatient rehabilitation facility
  • Hospital to an assisted living community
  • Hospital to a memory care community
  • Hospital to hospital transfers between levels of care
  • Return trips for follow-up appointments after discharge

For Families: What to Expect on Discharge Day

Call or text us as soon as you know discharge is likely, even before the exact time is set — an approximate window is enough to get started. Tell us the hospital and unit, whether your family member walks, uses a wheelchair or needs positioning support, and where they are going.

Our driver assists from the unit door or discharge entrance into the vehicle, secures any mobility equipment, and helps the patient inside at the other end rather than leaving them at the curb. If you cannot be present, we can confirm arrival by phone or text.

For Case Managers and Discharge Planners

Transportation should not be the reason a discharge slips to tomorrow. One call to Presence covers ambulatory, wheelchair, bariatric and BRODA / Traversa-level passengers, so your team is not working through a list of vendors to find a vehicle that fits the patient.

We confirm assignment, communicate delays before they become surprises, and close the loop when the patient reaches their destination. Recurring post-discharge appointment transportation can be scheduled at the same time as the discharge ride.

  • Same-day and next-day discharge transportation requests
  • Wheelchair and higher-support passengers handled by one provider
  • Facility accounts available so trips can be arranged without collecting payment at the curb
  • Pickup and drop-off confirmation back to the unit or coordinator
  • HIPAA-conscious communication and ADA-accommodating vehicles

Additional Support After Discharge

Some patients and families want more than a ride home on discharge day. Presence can arrange optional accompaniment during transportation following discharge, and when appropriate and arranged in advance, support that continues after arrival.

Options may include a non-clinical companion, a CNA, an MA, or a Nurse escort, depending on the patient's needs and staffing availability. This can be particularly valuable when family members cannot personally accompany a loved one home from the hospital.

Optional and available upon request for an additional fee, based on the level of support requested and availability.

Frequently Asked Questions

Can you pick someone up the same day they are discharged?
Often yes. Same-day discharge transportation is a routine request for us. Call or text as early in the day as possible and we will tell you exactly what window we can hold.
Can hospitals and case managers arrange transportation directly?
Yes. Case managers, social workers, discharge planners and unit staff can arrange trips by calling dispatch, and facilities can set up an account so transportation is billed to the organization instead of the patient.
Do you provide wheelchair transportation for discharge?
Yes. Wheelchair discharge transportation is one of our most requested services, including power chairs and bariatric equipment.
Can you take a patient from the hospital to a skilled nursing or rehab facility?
Yes. We regularly move patients from hospitals to skilled nursing, rehabilitation, assisted living and memory care destinations across DFW.
What if the discharge time changes?
That is normal. Keep us posted and we will adjust. Because dispatch is local and staffed 24/7, a shifted discharge time is handled by a phone call rather than a rebooking process.
Do you transport patients who need more than a standard seat?
Yes. Bariatric transportation and BRODA / Traversa transport chairs are available for patients who need greater positioning and transfer support. Presence provides non-emergency transportation only; patients requiring active medical intervention en route need an ambulance.
