Reducing Missed Appointments and Discharge Delays
Missed appointments and late discharges are usually transportation problems wearing a clinical costume. A patient who cannot get to dialysis ends up in the emergency department; a discharge that waits on a ride occupies a bed another admission needs.
Our Transitional Care Management program exists for exactly that window — the 30 days after discharge, when a missed follow-up visit is the difference between recovery at home and a readmission.

Some patients need more support than transportation alone. Presence can help coordinate optional non-clinical accompaniment or appropriately qualified clinical escorts, including CNA, MA, or Nurse support, based on the patient's needs and availability.
