Hospitals typically require a patient to have a ride arranged before they'll finalize discharge, but discharge timing is often unpredictable — a doctor's final rounds, paperwork, or a pharmacy delay can push the actual departure time later than expected. Patients recovering from surgery, illness, or a fall also often can't safely ride in a standard car, especially if they're weak, in pain, or need to stay in a wheelchair.
Planning discharge transportation ahead of time reduces stress for the patient and the family, and it helps avoid a hospital bed sitting occupied longer than necessary while everyone scrambles for a ride.
How the Process Typically Works
- The hospital's case manager or discharge planner confirms the patient is medically ready to leave
- The family or case manager contacts a transportation provider to schedule the pickup, ideally a day in advance
- The provider stays in communication as the discharge time shifts, since hospital timelines rarely run exactly on schedule
- A driver arrives with the appropriate vehicle — wheelchair van, stretcher-capable vehicle, or standard accessible vehicle depending on the patient's condition
- The patient is assisted from the hospital room or discharge area to the vehicle, and then from the vehicle into their home or the next care setting
Home, Rehab, or Skilled Nursing?
Not every discharge goes straight home. Some patients are discharged to an inpatient rehabilitation facility or skilled nursing facility for continued recovery before returning home. Make sure the transportation provider has the exact destination address and knows whether the drop-off is at a hospital-style entrance, a residential home, or a facility with specific check-in procedures.
If a patient will need to return to the hospital or clinic for a follow-up appointment soon after discharge, it's worth asking the transportation provider whether they also handle that follow-up trip so the same team is familiar with the patient's situation.
What to Prepare Before Discharge Day
- Confirm the exact discharge destination — home, rehab facility, or skilled nursing
- Ask the hospital for an estimated discharge time, and share it with the transportation provider along with a note that timing may shift
- Note any equipment needed for the ride: wheelchair, stretcher, oxygen, or other medical equipment
- Arrange for someone to be at the destination to receive the patient, especially if returning home alone
- Have a phone number ready so the provider and hospital staff can coordinate directly if the discharge time changes
